Looking for a fishing log

My Bass club used to have a link to a website that you could print off fishing logs (journal) to keep track of the fishing "adventures". They recently changed the website and now they no longer have it Does any one have a website that has a good log (journal). One with detail!

Here's one we came up with. I keep some in the boat or fill them out when I get home. Over the winter, I'll type them up and save them on the computer.

Water Conditions:
1.) Water temperature:
2.) Water Clarity:
3.) Water Level:

Weather Conditions:


Location: (River or Lake and specific part of each. ex: “Glen Lake, North West corner)

Tackled & Techniques used: (ex: fished Frog for slop Bass. Caught several between 2-3 lbs w/ largest being 4.2 lbs)
